Population Health Management: Engaging Primary Care Providers to Reduce Unwanted Clinical Variation and Support ACO Cost and Quality Goals: A Unique Provider-Payer Collaboration
This QURE study included primary care physicians and advanced practice professionals from practices across the state of Connecticut. The paper highlights strong increases in care standardization, improvements in patient-level quality measures, and estimated savings of nearly $5M from reductions in unneeded testing and specialist referrals. Note that in 2017, ProHealth was one of the top MSSP ACOs, earning $8.6M.
